About Your Symptoms

• I can walk more than 100 meters continuously
• Leg strength is preserved
• No urinary or bowel problems
• Symptoms are stable (not worsening)

All four are Yes -- Continuing conservative treatment may be an option Any are No -- Consider options including surgery

About Your Treatment History

• I have not yet had surgery
• I have not tried nerve block injections
• I have not had sufficient rehabilitation

Any are Yes -- There are still conservative treatment options to try

If You Have Had Surgery

• I had surgery but pain remains
• I have been told reoperation is difficult
• Chronic pain continues

Any are Yes -- SCS may be an option
